Apply for a service dog — at no cost to you.
A trained service dog is a significant community investment. We ask every applicant to share why they are ready for this responsibility and how they will honor the donation that makes their placement possible.
Eligibility
- You are a resident of the United States and at least 18 years old (applications for minors are reviewed with a parent or guardian).
- You have a documented physical, psychiatric, or neurological disability that a trained service dog could help with.
- You understand that each placement involves significant training costs funded by our donors, and you are prepared to treat this gift with respect and gratitude.
- You are able and willing to travel to our trainers in Central Ohio for an in-person evaluation and team training before placement.
- You can meet a dog's daily needs — food, exercise, grooming, and routine veterinary care — for the life of the dog.
- Your home environment is stable, safe, and welcoming to a working dog.

- We review your application and reach out with questions.
- An in-person meeting and evaluation in Central Ohio.
- Matching with a dog whose task work fits your needs.
- Hands-on team training together, then placement — free of charge.
